چکیده

We report unusual subtrochanteric non-union and metalwork failure after an index procedure of a trochanteric entry-pointlocked cephalomedullary nailing. We treated this non-union case successfully with the exchange of a thicker and longer intramedullary (IM) nail, supplemented by autologous iliac crest bone graft and bone morphogenic protein 7 (BMP-7). We think the success is due to the optimisation of the mechanical environment (the revision of fixation) along with the enhancement of the biological pathways of bone healing. A single stage surgical revision can be used for recalcitrant atrophic non-union fracture with implant failure.

Introduction: Close reduction and intra medullary nailing has been established as the treatment for femoral shaft fractures with a non::union:: rate of 0.9-1.1 % though, it is still controversial. Currently, the most accepted method for treatment of non ::union:: is exchange nailing. Therefore, this study aimed to investigate treatment of femoral shaft fractures non ::union:: via 2 methods of e...

Subtrochanteric fracture non-unions with implant failure managed with the "Diamond" concept.

BACKGROUND Subtrochanteric femoral non-unions in the setting of failed metalwork pose a challenging clinical problem. This study assessed the clinical outcome of patients treated according to the principles of the "Diamond" concept. METHODS Between 2007 and 2011 all patients presented with a subtrochanteric atrophic aseptic non-union in the setting of metalwork failure (broken cephalomedullar...

Outcome of treatment of nonunion tibial shaft fracture by intramedullary interlocking nail augmentated with autogenous cancellous bone graft.

To assess results of operative treatment of non union fracture shaft of Tibia by intramedullary interlocking nail augmented with autogenous cancellous bone graft in our setup. A total of 25 nonunion tibial shaft fractures were evaluated among which 20 cases were male and 5 female with the mean age 31.84 years. Hypertrophic non-union were 14 and atrophic non union were 11. Upper one third of tib...

Use of femoral nail with spiral blade in subtrochanteric fractures.

OBJECTIVE The aim of this study was to evaluate the outcome of acute subtrochanteric fractures managed with intramedullary nail and spiral blade fixation of the proximal fragment. METHODS Charts of 33 patients (17 males and 16 females) with acute subtrochanteric fractures operated with intramedullary nail and spiral blade at our institution between March 2006 and February 2011 were retrospect...

Treatment of Proximal Femur Osteomyelitis Occurred after Proximal Femoral Nail Antirotation Fixation, with Antibiotic Cement-coated Tibia Intramedullary Nail: A Case Report

Antibiotic cement-coated intramedullary nails maintain a locally high antibiotic concentration while contributing to bone stability. We present a case of femoral subtrochanteric fracture in a patient with an infected nonunion who was successfully treated for an infection and nonunion using an antibiotic cement-coated tibial intramedullary nail. A 79-year-old woman with a right femoral subtrocha...
